If you received a text, email, or letter regarding the announcement that Dr. Glenda Weeman will be closing her practice, you may wonder what you should do next to continue your medical care.

If you have a scheduled follow-up appointment between now and December 31, 2025, you may keep your appointment.  Dr. Weeman will continue to see patients on a regular schedule.  If you find a new provider and no longer need your appointment, please call and cancel your appointment.

We strongly suggest that you look for a new provider as soon as possible so you do not have a gap in care.


TRANSFER OF MEDICAL RECORDS

UNTIL December 31, 2025, you can initiate a records transfer of your medical records to a new provider OR receive a personal copy.

1. New Provider  - your new provider will usually have a form to fax to us to request records to be sent to them directly.  When we receive this form via fax, we will send your new provider a copy of your medical records via fax.

2. Personal Copy - you will receive either a printed copy of your records or we will provide a way to retrieve a copy of your records electronically.

We will email or text you instructions to download your medical records file.  You can then give these records to a new provider when you find one. 

NOTE: When you receive printed records or the link to download your medical records file, you have officially transferred out (permanent termination of services).

Between JANUARY 1, 2026 UNTIL MARCH 31, 2025 you will only be able to retrieve your medical records by requesting them electronically.  Paper copies will not be available.  We will email or text you instructions to download your medical records file.

After March 31, 2026, there will be a charge to download an electronic copy of your records.
