SUGGESTED PROVIDER LIST
ADDITIONAL PROVIDER SUGGESTIONS
If you received a text, email, or letter regarding the
announcement that Dr. Glenda Weeman will be closing her
practice, you may wonder what you should do next to continue your
medical care.
If you have a scheduled
follow-up appointment between now and December 31, 2025, you may
keep your appointment. Dr.
Weeman will continue to see patients on a regular schedule.
If you find a new provider and no longer need your
appointment, please call and cancel your appointment.
We strongly suggest that
you look for a new provider as soon as possible so you do not have a
gap in care.
TRANSFER OF MEDICAL RECORDS
UNTIL December 31, 2025,
you can initiate a records transfer of your medical records to a new
provider OR receive a personal copy.
1. New Provider - your new provider will
usually have a form to fax to us to request records to be sent to
them directly. When we receive this form via fax, we will send
your new provider a copy of your medical records via fax.
2. Personal Copy - you will receive either a printed copy of
your records or we will provide a way to retrieve a copy of your
records electronically.

Between JANUARY 1, 2026 UNTIL MARCH 31, 2025
you will only be able to retrieve your medical
records by requesting them electronically.
Paper copies will not be available.
